Health Systems Improvement and Sustainability (HSIS) Level Initiative

It has been estimated that 10 to 30 per cent of healthcare worldwide is wasteful, of little or no value, or harmful to patients. Using scarce resources to deliver low value care is an important health system concern as resources used for that care could be allocated to care associated with greater benefit.

The HSIS Working Group 2, led by Health Translation Queensland in partnership with the Australian Centre for Health Services Innovation (AusHSI), has developed a practical toolkit to empower health professionals to make better decisions about clinical care: Moving towards high value care toolkit: Identifying and addressing low value care.

HSIS Working Group 2 was one of three working groups established under the Health Systems Improvement and Sustainability (HSIS) National System Level Initiative, which aims to develop  workforce and models of care that improve the healthcare system and its sustainability.  HSIS Working Group 2’s goal was to address change and improvement of clinical practice.

The toolkit

Moving towards high value toolkit: Identifying and addressing low value care aims to drive safer, higher quality patient care by supporting health professionals to recognise and reduce low-value care practices—health interventions or models of care that provide little or no benefits, may cause patient harm or yield marginal benefits at a disproportionately high cost.

The toolkit has three focus areas:

  • Planning: selecting strategies to de-implement low-value care, including how to engage relevant stakeholders from the outset
  • Action: including the facilitation, leadership and monitoring of de-implementation
  • Sustaining better care: keeping up the momentum beyond the initial change and embedding new standards of higher-value care into business as usual.

With partners and investigators from across AusHSI, Metro North Hospital and Health Service, Metro South Hospital and Health Service and Children’s Health Queensland, the project team gathered input on the toolkit from various stakeholders, including clinical experts (medical, nursing, allied health professionals), healthcare consumers, implementation scientists, health economists, healthcare leaders and managers.

The toolkit has been developed to support staff in hospitals and health services to optimise the value of care provided to patients. The toolkit includes questions clinical teams can ask themselves to identify the value of care, clinical scenarios, time and cost mapping tools, and resources to support behaviour change.
